Hong Kong Identity Card / Passport
Membership Certificate, Notice of Acceptance, Notice of Participation or Annual Benefit Statement showing the Name of Original Scheme, Scheme Member Account Number or the Employer’s Identification Number (if applicable)

Please note, if you would like to choose direct debt, please fill in and return “Application of Participation Form for Self-Employed Person” which can be download from “Download Centre” of our website.
